Read this first: published fee schedules are the fee-for-service baseline. In heavily managed-care states, plans negotiate their own rates against that baseline — so treat these numbers as the floor of the conversation, not your revenue model. Rates also vary by provider level and modifier (our CPT reference explains the code structure). Always bill from the current official schedule, linked on each page.
Across the published schedules the spread is enormous: an hour of the core direct-therapy code 97153 (treatment by protocol) runs from $39.60 in West Virginia and $40.00 in Alabama to $127.80 in Alaska — the same billed service paying three times as much depending on which state line a family lives on. Twenty-two states publish enough to compute an hourly figure. Nine publish a rate but no time unit, and we do not assume one. Two — Idaho and Maine — do not use the CPT codes at all, billing ABA under their own H-code schedules instead. Three publish no fee-for-service rate whatsoever, including California, which is still establishing rates and pays "by report" in the meantime. Each of those is a finding rather than a gap.
