Read before using these numbers

One of the two previously cited URLs is DEAD. https://dhhr.wv.gov/bms/Public%20Notices/Documents/Chapter%20519/Chapter%20519.23%20ABA%20FINAL%20for%20Comment.pdf returns HTTP 404, and it was in any case a draft posted for public comment, not a rate schedule. The other cited URL, the BMS fee schedules index, is live and links the correct file. The adopted policy now lives at https://bms.wv.gov/media/40119/download?inline (Policy 519.23 Applied Behavior Analysis).
97157, 0362T and 0373T are not on West Virginia's ABA fee schedule and are not covered in Policy 519.23.
The unit comes from a second official document, not the fee schedule. The fee schedule itself has no unit column. BMS Provider Manual Chapter 519.23 Applied Behavior Analysis, Effective Date April 1, 2020, prints "Service Unit: 15 minutes" against each of 97151, 97152, 97153, 97154, 97155, 97156 and 97158.
The document has no overall effective date. Each row carries its own Code Eff Date (04/01/2020 or 04/01/2021) and a Code Term Date of 12/31/2078. The rates have not moved since 2021.
The same schedule prices five HCPCS codes alongside the CPT codes — H0031 $120.00, H0032 $29.14, H2012 $29.14, H2014 $11.79 (U4 $5.50, U5 $2.50) and H2019 $29.14. These are not ABA CPT rates and should not be mixed in.
Policy 519.23 sets a combined limit across 97151-97156 and 97158 of 40 hours per week and eight hours in any 24-hour period.
The only modifiers on the schedule are U4 and U5 on H2014; none of the CPT rows carries a modifier or a provider-tier variant.
